If you are a Louisiana resident who is a surviving spouse of a member of the U.S. Armed Forces, including the Louisiana National Guard or Air National Guard, who was killed in action while in a combat zone, you may purchase a combination hunting and fishing license.

To qualify for this license, you must submit the following:

• Copies of the DD 1300 form
• The death certificate
• Your valid Louisiana driver’s license or Louisiana identification card
  • You must have also lived in Louisiana continuously for at least the past 6 months.
